In Italy the cup is played at the higher-ranked side's ground, always. No coin toss, and no second leg until the semi-finals: if you're in Serie C and you draw Inter, you go to San Siro and that's where your tournament ends or your legend begins. That one rule explains why upsets here are worth double.
It starts on 8 August with four ties between Serie B and C sides — Catania at Vicenza, and so on down the list — and a week later the twelve unseeded Serie A clubs come in. The big clubs enter in December, at the last 16, once the draw has thinned itself out.
The final is at Rome's Olimpico and carries a Europa League place. Juventus lead with 15 titles and Inter and Roma share second on 9, but the format guarantees that every round contains a team that shouldn't still be there and is.
The village that won the first one
On 16 July 1922, at the Campo di Leo in Vado Ligure, Virgilio Levratto poked the ball home in the 118th minute and a Riviera village lifted the first Coppa Italia. Vado, then playing in Promozione, had beaten Udinese 1-0 in extra time. They remain the only champion never to have set foot in Italy's top division.
The trophy then almost disappeared. It was not contested between 1923 and 1925, nor between 1927 and 1934, nor between 1944 and 1957: it took more than three decades of stops and restarts before the cup became an annual fixture. Of the 79 editions called, only 78 trophies have ever been handed over, because the 1926/27 tournament was abandoned unfinished.
Once it settled, the usual names took charge. Juventus have 15 titles, four of them in a row between 2015 and 2018, a run nobody has matched. Inter reached 10 in 2026 and moved clear of Roma, who stay on 9; behind them come Lazio (7), Fiorentina and Napoli (6), Torino and Milan (5). And still the trophy tastes of the second tier: Napoli won it in 1962 while playing in Serie B, and Vicenza in 1997 were the last provinciale to lift it.

Marušić turned the ball into his own net in the 14th minute and the final was broken before it began. Twenty-one minutes later Lautaro Martínez made it 2-0 and Inter lifted their tenth Coppa Italia at the Olimpico, in a rerun of the 2000 final that Lazio had won. With the Scudetto already secured, the double carried a little-known administrative consequence: the Supercoppa place reserved for the cup winner passed to the beaten finalist.
The road there gave far more than the final scoreline suggests. In the quarter-finals Atalanta ran straight over Juventus (3-0), Lazio knocked out Bologna on penalties after a 1-1, Inter saw off Torino (2-1) and Como produced the night of the tournament: 1-1 at the Maradona and 7-6 from the spot. In the semi-finals Como held Inter to 0-0 at home and only went out in Milan (3-2); Lazio and Atalanta drew 2-2 and then 1-1, and settled the ticket in another shootout, in Bergamo.

Eight clubs waiting for December
Inter defend the trophy as top seeds and will not play a match until December. Waiting alongside them are Napoli, Roma, Como, Milan, Juventus, Atalanta and Bologna: the top eight of the ranking drawn from last season's Serie A go straight into the round of 16, between 2 and 16 December, and they go in at home. Como sit fourth, ahead of Milan and Juventus.
The bracket is already split in two, and that shapes the whole winter. On the left: Inter, Bologna, Milan and Como. On the right: Roma, Juventus, Atalanta and Napoli. Translated: a Milan derby can only happen in the semi-finals, and Inter and Juventus can only meet on 19 May. If the favourites hold, the last 16 points to Inter-Genoa, Bologna-Lazio, Milan-Torino, Como-Parma, Roma-Cagliari, Juventus-Sassuolo, Atalanta-Udinese and Napoli-Fiorentina.
The other twelve Serie A clubs start in August, and four of them do not yet know who they will face. Genoa await the winner of Ascoli-Potenza, Parma the winner of Vicenza-Catania, Cagliari the winner of Arezzo-Union Brescia and Fiorentina the winner of Benevento-Ravenna. Lazio, beaten finalists three months ago, are back on 16 August against Mantova.
Seven rounds and a bracket already written
Forty-four clubs: the 20 of Serie A, the 20 of Serie B and four from Serie C. There is no round-by-round draw. The tabellone is ordered top to bottom by what each club did the previous season and published in full before a ball is kicked, so in August everyone already knows the road they would have to walk to Rome.
There are seven rounds. Preliminary (4 ties, 8-9 August), trentaduesimi (16, 14-17 August), sedicesimi (8, 2 September), round of 16 (8, 2-16 December), quarter-finals (4, 3-10 February) and the semi-finals, the only two-legged round: first leg 3 March, return 21 April. The final is on Wednesday 19 May 2027 at Rome's Olimpico.
The detail that changes predictions most arrived in 2024. Since then there is no extra time in the single-match rounds: if the score is level on 90 minutes it goes straight to penalties, and that rule holds all the way through the quarter-finals. Extra time survives only in the semi-finals and the final. Whoever lifts the trophy enters the Europa League league phase and takes a place in the Supercoppa Italiana; win the Scudetto as well and that place passes to the beaten finalist.
